Medicaid Health Homes
Our Health Homes program expands upon Diaspora Community Services’ years of success assisting family members manage and coordinate their medical and behavioral care. As Health Homes care coordination providers we work collaboratively with Medicaid-eligible individuals who have one or more of the following chronic illnesses:
• Asthma
• Diabetes
• Heart Disease
• HIV
• Hypertension
• Obesity
• Mental Health
• Substance Abuse
Utilizing a client-centered, family–focused approach DCS’ care management service model works to remove the barriers and improve access to primary and behavioral health care. Collaborating with the individual, their medical and behavioral health providers, and other community based network providers we address the individual’s needs and goals in a comprehensive manner. Our Care Management team coordinates these individualized services (housing, entitlement benefits, medical case conferences, health education/promotion, advocacy, etc.) to assure our community members stay healthy, reduce emergency room visits and inpatient services. Diaspora continues to provide culturally sensitive health promotion to work with individuals in promoting positive health outcomes.
